Contestant Application -- to be completed by a person seeking or renewing licensure to participate as a contestant in boxing, kickboxing, amateur mixed martial arts, professional mixed martial arts, wrestling or elimination events.

Non-contestant Application -- to be completed by a person seeking or renewing licensure to participate as a judge, trainer, manager, referee, timekeeper, second or event staff in boxing, kickboxing, mixed martial arts or wrestling.

Physical Exam -- to be completed by a person seeking or renewing licensure as a contestant or referee in boxing, kickboxing or mixed martial arts. This form is also to be completed by a person seeking licensure or renewal as a wrestling contestant or referee if 45 years of age or older or if the person has not held a license in the previous two years.

Show Notice Form -- to be completed by a promoter prior to holding or advertising an event.

Event Report -- to be completed by a promoter after holding any type of event.

New Promoter Packet -- to be completed by those seeking licensure as a promoter in boxing, kickboxing, mixed martial arts or wrestling. This packet includes a checklist of requirements, the promoter application and the promoter bond form.

Medical Provider Application -- to be completed by a person seeking or renewing licensure to participate as a physician or healthcare professional in boxing, kickboxing, mixed martial arts, wrestling or elimination events.

Medical Release Form -- to be completed by a licensee who has been placed under medical suspension at the time the licensee is cleared to participate.

Complaint Form -- to be completed by any person that has a complaint, issue or recommendation to the Kentucky Boxing and Wrestling Commission.

Boxing Federal ID Application -- This form is used to obtain a Federal ID Card for Professional Boxing. Federal law requires obtaining this ID before being approved to compete in professional boxing. Obtaining a Federal ID does not mean you are qualified to participate in professional boxing. It is only to verify your identity and that you are registered with the Boxing National Registry. Once this ID is issued you are no longer eligible to compete in amateur combat sports. Please make sure you have finished your amateur career before submitting this application.​
